CUSTOM PATCH ORDER FORM

Please include as much information about your order as possible. If you would like specific size, shape, color, font, etc.

You will receive an email approximately 24-48 hours after your order has been submitted with a digital mock up and price quote for your order.

Depending on the size and complexity of your order, you may be asked to pay a deposit up front before work begins.

Turn around time is typically 1-3 weeks. Orders are filled on a first come first served basis and you will be notified of current turn around time in your quote email.

When your order is complete you will receive an email to notify you that a custom listing as been created for you here in the web store.
